How to Create a Form And Add It to a Page

Within the back end of the system, you can create a form that can be used on any page on the site.  Using these forms, you can create your own landing pages and once a form is completed, have those leads feed into the back end and follow the lead rotation you have set up.

To do this, you'll need to:
1) Create the form
   a) Refer to the attached spreadsheet for Supported Field Keys
2) Add the form to a page
